Before you return your equipment for service, have you:

When you are ready to return your equipment, please:
- Complete the following RMA form
- Ship the item in its original box
- NOTE: A reboxing fee may be applied to items not returned in their original boxes
- Carefully pack the item to prevent damage
- Include your master disc for testing
- Write the RMA number on the shipping label, NOT on the box
- Include a note detailing the problems with your equipment or services to be performed
